Verifying your daily self-assessment for COVID-19 is done by accessing Workday through the Workday App.
To access Workday:
- Navigate to myapps.microsoft.com. Login with your credentials for Office 365.
- You may be prompted to authenticate via Multi-Factor Authentication or to set up the Multi-Factor Authentication if you have not done so previously.
- Locate and click the blue Workday icon from the available applications list.
- When you click the Workday app, you will be prompted to validate the Multi-Factor Authentication again to log into Workday. This is done to protect the sensitive information stored in Workday.
- You will be logged into Workday.
Filling out the certification:
- Click anywhere on the Daily COVID-19 Certification announcement to open a pop-up window.
- Click the "Daily COVID-19 Certification" link.
- Read and fill out the form by checking one of the following options:
- If yes, you certify that you have not experienced any of the following within the previous 14 days:
- Any new symptoms of coughing, shortness of breath, difficulty breathing, sore throat, diarrhea, vomiting, headache, nausea, fever, chills, or muscle aches.
- New loss of taste or smell.
- Close contact (within 6 feet for 15 or more minutes) with a person confirmed as having COVID-19.
- Completed symptom- or exposure-based COVID-19 testing with results pending or positive.
- No, if you should not be on campus based on the above criteria.
- Not applicable, if you will not be working on campus today.
- If yes, you certify that you have not experienced any of the following within the previous 14 days:
- Click OK.
